Log My Care is a user-friendly care management platform designed to support care providers in delivering more Person-centered care. Its interface aims to streamline operations and enhance the quality of service provided.

The Carer App serves as the primary tool for care teams to carry out their duties and interact with service users efficiently.

This application works in conjunction with the Care Office module to constitute an integrated care management system.

Once your account is established, the Carer App provides the following functionalities:

• Document and log the care activities performed, including nutritional support, clinical interventions, activities, incidents, and handovers. The app also allows uploads of photos and short videos to enrich care records.
• Access service users' care documentation securely from any location.
• Review and respond to daily tasks assigned by your care management team.
• View detailed care and support plans that have been developed within the platform by your Care Manager.
• Utilize advanced features such as Positive Behavioral Support (PBS), goal setting, and progress tracking.
• With the Rostering add-on, carers can view upcoming schedules, manage planned absences, and clock in/out with ease.
